Apply for a Permit

Apply for a Well Permit   
From this site you will be able to apply for a well permit and a well repair permit. Issuance of a permit will only be authorized when payment is made and a site plan of the property is submitted. Payment can be made in person or on the phone by calling 641-7613. Submittal of a site plan can be faxed, mailed, delivered in person, or emailed to: environmentalhealth@co.guilford.nc.us
